 Why must I bring my insurance card every time I come to LRH?
For your protection and for the hospital to bill your insurance correctly it is important to have correct and updated insurance information; your current insurance card provides us with the information we need to serve you better.
   
 Why do I need a photo I.D?
To protect our patients from identity theft, and to comply with Federal Law we must verify the identity of each patient; a photo I.D. is the best way to accomplish this task.
   
 Why do I have to answer the same questions every time I register?
Because patient and family information can change we must verify all information currently in our records. This allows us to correctly bill insurance and keep your records accurate.
   
 What is the Consent for Treatment form?
The Consent For Treatment form gives Littleton Regional Hospital, its physicians, employees, and all individuals involved in your care consent to administer care or treatment to evaluate and treat your injury or illness. This includes inpatient or outpatient medical services, general duty nursing, surgical procedures, emergency services, laboratory procedures, x-ray examinations, anesthesia and other treatment(s) under general and specific instructions of your physician(s).
   
 What is Durable Power of Attorney (DPOA)?
This is a written, legal document that allows you to appoint a person to make health care decisions for you when you are not capable of making the decision. Littleton Regional Hospital requires that you or your POA provide this document upon admission, when you are unable to sign the Consent For Treatment form.
